Transcript Request Forms

If you need the Counseling Office to send your transcript anywhere, you must fill out a transcript request form.  Below are two forms, one for current students and one for former students.  There is a 3.00 processing fee for former students per request.  Forms must be filled out and sent (along with processing fee if applicable) to:
